Does your congregation or faith-based organization have a desire to live a heart-healthy life? If so, join 100 Congregations for Million Hearts®!
100 Congregations for Million Hearts® is a faith-based program designed to help guide your members toward a heart-healthy lifestyle. Through this initiative, we aim to combat two of the leading causes of death in the United States: heart disease and stroke. The goal of 100 Congregations for Million Hearts® is to enlist the support of faith based organizations to help raise awareness and prevent cardiovascular disease and focus on hypertension control.
With your commitment, we ask that your congregation designate a Million Hearts® advocate from among your membership to serve as a resource for heart health information.
In addition to the Million Hearts® advocate, congregations are being asked to:
- Establish and/or strengthen relationships with local experts and resources (community pharmacists, community health centers and community health workers);
- Disseminate messages about the importance of and action steps to hypertension control;
- Promote Heart Health Mobile (an app that helps you determine and reduce your heart attack and stroke risk) and;
- Distribute blood pressure tracking wallet cards [PDF-921K].
