American College of Clinical Pharmacy (ACCP)

The American College of Clinical Pharmacy (ACCP) is a professional and scientific society that provides leadership, education, advocacy, and resources enabling clinical pharmacists to achieve excellence in practice and research. ACCP's membership is composed of practitioners, scientists, educators, administrators, students, residents, fellows, and others committed to excellence in clinical pharmacy and patient pharmacotherapy. As the leading national professional organization advocating for the delivery of comprehensive, team-based medication management across the entire health care delivery system, ACCP works directly with physician organizations, inter-professional collaboratives such as the patient-Center Primary Care Collaborative, the Institute of Medicine, and other public and private entities to ensure that patients and their families are at the center of the practice and purpose of the nation's clinical pharmacists.

ACCP will support the Million Hearts™ initiative by working with the Department of Health and Human Services and other health professional organizations to develop tools and resources that support team-based care in the management of cardiovascular disease, disseminating these tools and other best practices and outcomes research via member education and its member-driven Practice and Research Networks (including Cardiology PRN), and promoting the highest quality of medication-related care for patients with cardiovascular disease.
