Success Story: Large Health System Implements Medication Adherence Practices

In previous rounds of the Hypertension Control Challenge, Million Hearts® established a benchmark of 70% hypertension control for applicants’ adult populations. This 2012 success story reflects the earlier benchmark.
This health system in Denver helped improve hypertension control rates for more than 100,000 people by making sustainable, long-term changes in its patient protocols. Kaiser used electronic health records, created patient outreach plans, and collaborated among team members to overcome barriers to blood pressure management.
