“Heart-Healthy Steps” Campaign Partner Toolkit
Million Hearts® collaborated with the CDC Foundation to develop a national cardiovascular disease prevention campaign to promote heart-healthy steps for primary and secondary prevention.
The “Heart-Healthy Steps” campaign encourages adults, particularly those age 55 and older, to take charge of their heart health with small steps—like working with health care professionals, getting active, and eating healthy.
This toolkit includes messages and materials to encourage adults educational information and motivation for practicing heart-healthy habits that can help lower their risk for heart disease and stroke.
You can use the assets included below and/or direct people to HeartHealthySteps.org.

With a healthy heart, you can do more of what you love — but it’s not always easy to break old habits or make healthy changes. The “Heart-Healthy Steps” campaign encourages adults, particularly those age 55 and older, to get on track with small steps—like scheduling their medical appointments, getting active, and eating healthy—so that they can improve their heart health. We are happy to join with the CDC Foundation, the Million Hearts® initiative, and organizations across the nation in sharing these important messages. Learn more at HeartHealthySteps.org.
