Million Hearts^® recognized the @Promise Community Health Center in Sioux Center, Iowa, as a 2024 Hypertension Control Champion for achieving a hypertension control rate of 80% in 2023!

Promise Community Health Center serves a rural community with a large Spanish-speaking population. In 2022, Promise had a hypertension control rate of 73%. Inspired to do better, Promise formed a work group to develop new policies and procedures to improve hypertension control. Their approach includes:

• Frequent follow-ups: Staff follow up with patients with uncontrolled hypertension monthly instead of every 3 months, as is common in other settings. Care teams also stay in touch with patients with controlled hypertension to help them maintain that control.
• Health care at home: Staff use self-measured blood pressuring monitoring and remote patient monitoring systems that let patients check their own blood pressure at home, reducing their travel and costs. Staff also provide CDC handouts with tips on taking accurate blood pressure measurements and on maintaining a low-sodium diet.
• Coaching for a heart-healthy lifestyle: Staff use a nurse health coaching program that lets them guide and encourage patients to make lifestyle changes for heart health.
• Teamwork, teamwork, teamwork: Staff fulfill all aspects of hypertension management, from offering blood pressure checks to providing medical assistance and scheduling services in both English and Spanish. Collaboration between physicians, medical assistants, pharmacists, and nurse health coaches ensures a comprehensive experience.

Promise Community Health Center is dedicated to ensuring patients get care regardless of language, background, or income level. Whatever barriers patients may face—long work hours with little time off, living far from the clinic, or trouble affording heart-healthy foods—the team is committed to finding solutions that work for the patients they serve.
